Abstract Introduction Liposarcomas are malignant neoplasms that originate in fatty mesenchymal tissue and the most common histological subtype of soft sarcomas. They account for 1% solid tumors adults located mainly extremities, with retroperitoneum being second frequent location. Given rarity aggressiveness these tumors, it is advisable to establish a multidisciplinary approach towards them order improve prognosis patients. Clinical case Patient who was operated upon on scheduled outpatient basis left inguinal hernia. During surgery, preperitoneal tumor identified removed revealed high-grade dedifferentiated liposarcoma. In surgical act, mass about 10 centimeters length found, lodged pelvis contacting sigma, which extended into duct scrotal pouch. Finally, compartment surgery orchiectomy end-to-end colo-sigmoid anastomosis were performed. Discussion Routine examination masses contained hernial sacs recommended since, despite their infrequency (<0.1%), cases liposarcomas have origin canal been described. Surgery retroperitoneal sarcomas must be compartmental, requires inclusion viscera close resection. Complete resection R0 only potentially curative treatment.
